What Heart Conditions Might Confusion or Brain Fog Indicate?
Brain fog and confusion are usually caused by things that have nothing to do with the heart, such as poor sleep, stress, dehydration, an underactive thyroid, certain medications, or simply doing too much. But the brain depends entirely on a steady supply of oxygen-rich blood, and it receives around 15 percent of everything the heart pumps out. When the heart is not pumping efficiently, or is beating irregularly, blood flow to the brain can fall, and the result can be difficulty concentrating, memory lapses, mental slowness, or a feeling of being not quite with it. This is why confusion and brain fog can, in a smaller number of cases, be a sign of an underlying heart condition rather than a purely neurological or lifestyle problem.
The heart conditions most commonly linked to brain fog and confusion are atrial fibrillation, heart failure, low blood pressure, and coronary artery disease, all of which can reduce the amount of blood reaching the brain. In our experience at our Warwick Park rooms in Tunbridge Wells, the patients most often caught out are older adults whose gradual cognitive changes are assumed to be part of ageing, and younger patients whose brain fog is dismissed as stress or tiredness. From working with patients across Kent and East Sussex since 2006 at West Kent Cardiology Partnership, we have found that when brain fog appears alongside palpitations, breathlessness, dizziness, or an irregular pulse, a proper cardiac assessment frequently identifies a treatable cause that improves both the physical symptoms and the mental clarity.
Why the heart affects the brain
The heart and brain are closely linked through what is often called the heart-brain axis. The brain has almost no capacity to store energy, so it relies on a constant, second-by-second delivery of oxygen and glucose through the bloodstream. Any drop in the heart’s output, or any interruption to a steady rhythm, is felt quickly by the brain.
When blood flow to the brain falls slightly but persistently, the effect is not usually dramatic. Instead of a collapse or a stroke, people experience subtler symptoms such as poor concentration, forgetfulness, slowed thinking, and mental fatigue. In our experience, this is exactly the pattern that tends to be overlooked, because it develops gradually and is easy to attribute to something else.
Atrial fibrillation
Atrial fibrillation is the most common heart rhythm disturbance and one of the most important cardiac causes of brain fog. In atrial fibrillation, the heart beats irregularly and often less efficiently, which reduces the amount of blood pumped with each beat and lowers overall blood flow to the brain. Research has linked atrial fibrillation to a meaningfully increased risk of memory and thinking problems, even in people who have never had a stroke.
There is a second mechanism too. Atrial fibrillation increases the risk of small clots forming in the heart, which can travel to the brain and cause tiny, sometimes silent, areas of damage that accumulate over time.
In our experience, a significant number of patients we diagnose with atrial fibrillation mention in passing that their concentration or memory has not been what it was. From working with patients, we have found that restoring and maintaining a normal rhythm, where appropriate, often improves this mental cloudiness. In our experience, treating atrial fibrillation actively with rhythm control such as cardioversion or ablation works better than rate control alone for some patients troubled by brain fog, because restoring an organised rhythm tends to improve the steadiness of blood flow to the brain. The same irregular rhythm that causes brain fog can also produce the sensation of a racing or pounding heartbeat, which is often what first brings patients to see us.
Heart failure
Heart failure means the heart is not pumping as strongly as it should. Because the brain depends on good cardiac output, reduced pumping power leads to chronic underperfusion of the brain, and cognitive difficulty is common. Studies suggest cognitive impairment affects a substantial proportion of people with heart failure, ranging widely depending on severity.
Brain fog in heart failure rarely occurs alone. It is usually accompanied by breathlessness, fatigue, and swelling of the ankles. From working with patients, we have found that the combination of worsening mental clarity and increasing breathlessness on exertion is one of the more reliable signals that heart function has declined and needs review.
Low blood pressure and postural hypotension
If blood pressure is too low, or drops sharply on standing, the brain can be briefly starved of blood, producing light-headedness, confusion, and difficulty thinking. This is particularly common in older adults and in people taking blood pressure or heart medications. The same underlying mechanism of reduced blood flow to the brain also explains why some people feel dizzy or foggy when they stand up quickly.
In our experience, around 1 in 3 patients we assess for these symptoms are taking at least one medication that lowers blood pressure and could be contributing, and adjusting the dose or timing often resolves the problem.
Coronary artery disease and after a heart attack
Narrowed coronary arteries reduce the efficiency of the heart and are associated with a higher risk of cognitive decline over time. Cognitive changes are also well recognised after a heart attack, partly due to the event itself and partly due to the underlying coronary disease. Managing these risk factors early is important not just for the heart but for protecting long term brain health.
Common non-cardiac causes
Most brain fog is not cardiac. Frequent causes include poor or disrupted sleep, stress and anxiety, dehydration, an underactive thyroid, anaemia, low vitamin B12, the after-effects of viral illness, perimenopause, and medication side effects. From working with patients, we have found that the clearest pointer towards a cardiac cause is whether the brain fog is accompanied by physical symptoms such as an irregular pulse, breathlessness, chest discomfort, or dizziness. Brain fog in isolation, with an otherwise normal heart examination, is far more likely to have a non-cardiac explanation.
When brain fog is more likely to be heart related
Certain features should prompt a cardiac assessment. These include brain fog with a noticeably irregular pulse, brain fog with breathlessness or ankle swelling, brain fog with dizziness or fainting, and cognitive symptoms in someone with known heart disease or significant risk factors such as high blood pressure, high cholesterol, or diabetes. In our experience, patients with two or more of these features benefit from a baseline cardiac assessment such as our HeartScreen programme even when the changes seem mild.
How the cause is investigated
Assessment begins with a careful history and examination, including checking the pulse and blood pressure both lying and standing. The key initial tests are an ECG to look at rhythm, blood tests to check thyroid function, anaemia, and other reversible causes, and an echocardiogram to assess how well the heart is pumping. Because rhythm problems such as atrial fibrillation are often intermittent, ambulatory monitoring with a 24-hour ECG recording or a 7-day cardiac event recorder is frequently the most valuable investigation. When to seek urgent help
Sudden confusion is a medical emergency. Call 999 if confusion comes on suddenly, or with face drooping, arm weakness, or slurred speech, as these can indicate a stroke or transient ischaemic attack and need immediate treatment.
Conclusion
Brain fog and confusion are usually benign and unrelated to the heart, but because the brain depends so heavily on good blood flow, they can sometimes signal an underlying cardiac problem. The conditions most often responsible are atrial fibrillation, heart failure, low blood pressure, and coronary artery disease. The pattern that should raise concern is brain fog accompanied by an irregular pulse, breathlessness, dizziness, or known heart disease.
